New Delhi: A major political controversy is unfolding in Iran as reports from opposition-linked media outlets and Israeli sources claim that Supreme Leader Mojtaba Khamenei may be facing a serious health crisis. The reports have triggered fresh questions over Iran’s leadership structure, succession politics and the future direction of the Islamic Republic.

Mojtaba Khamenei, who took over as Iran’s Supreme Leader after the death of his father Ayatollah Ali Khamenei following US-Israeli strikes in February 2026, has remained completely away from public view since assuming the country’s highest political and religious position. According to reports, he has not appeared in any public event, delivered a televised address or interacted openly with government officials. Iranian state media has only released written statements attributed to him, without showing any live visuals or direct communication.

Reports Claim Health Condition Has Worsened

IranWire, an opposition-aligned outlet, reported that two sources allegedly close to President Masoud Pezeshkian’s administration claimed that Mojtaba Khamenei has not met any cabinet members since the military strikes that killed his father and other members of his family.

The sources claimed that concerns about his health have grown among senior officials in Tehran, with speculation that his condition could be extremely serious. However, Iranian authorities have not officially confirmed any illness, hospitalisation or change in his ability to perform his duties. Israeli media reports have also intensified speculation, with claims that Mojtaba Khamenei was recently taken to a hospital after his condition deteriorated. These reports have not been independently verified.

Why Mojtaba Khamenei’s Absence Matters

The Supreme Leader holds the most powerful position in Iran’s political system. Under Iran’s constitutional framework, the Supreme Leader controls key state institutions, including the armed forces, judiciary, security apparatus and major foreign policy decisions.

Any uncertainty regarding the Supreme Leader’s health could have major implications for Iran’s internal power balance and its relations with regional and global powers. The prolonged absence of Mojtaba Khamenei has raised questions about whether he is actively managing state affairs or whether decision-making has shifted towards other senior officials within Iran’s ruling establishment.

Injury Reports After US-Israel Strikes

Several foreign media reports have claimed that Mojtaba Khamenei suffered injuries during the attack that killed his father, Ayatollah Ali Khamenei, on February 28, 2026. Some reports alleged that he sustained serious injuries and has remained in a secure location due to security concerns. It has also been claimed that communication between him and senior officials has taken place through intermediaries.

Iranian officials, however, have rejected reports suggesting severe injuries and have maintained that the Supreme Leader continues to oversee important national matters. Iran’s Health Ministry had earlier described his injuries as limited, while government representatives have insisted that there is no disruption in the functioning of the country’s leadership.

Pezeshkian’s Statement Adds to Speculation

Iranian President Masoud Pezeshkian recently acknowledged that direct interaction with Mojtaba Khamenei has become difficult. The statement further fuelled discussions about the Supreme Leader’s current condition and ability to directly engage with government officials. The lack of public communication has become unusual for Iran’s leadership structure, where Supreme Leaders traditionally appear during major national events and address citizens on important political and security issues.

Regional Powers Watching Closely

The developments surrounding Mojtaba Khamenei’s health are being closely monitored across West Asia. Iran plays a key role in regional security, with influence over allied groups and ongoing geopolitical disputes involving Israel, the United States and neighbouring countries. Any leadership uncertainty in Tehran could impact Iran’s military strategy, diplomatic negotiations and regional policies.

No Official Confirmation From Tehran

Despite growing international speculation, Iranian authorities have not issued any official statement confirming that Mojtaba Khamenei is seriously ill or hospitalised. At present, the claims are based on reports from opposition-linked outlets, foreign media organisations and unnamed sources. The situation remains unclear, with global attention focused on whether Tehran provides any official update regarding the Supreme Leader’s health. The mystery surrounding Mojtaba Khamenei’s prolonged absence has now become one of the biggest questions surrounding Iran’s political future, as the country faces internal challenges and increasing international pressure.
